Request a Formstack Account
Formstack is a form building and data collection tool that can be used to accomplish a wide variety of tasks. There are three main areas of Formstack:
- Form Builder - Select different types of fields and menus to design your form
- Form Settings - Configure everything from notification emails to approval workflows and form security
- Submissions Database - View, edit, export or manipulate the data which has been submitted via your form
Other software for forms and surveys is Microsoft Forms which works with Microsoft Teams.
If you need to access existing forms you can indicate that within the form (or we can grant you access to them after your account is created). If not OIT will create you your own folder where you can create any forms you need.
Please keep in mind protected HIPAA data is not allowed in formstack, all forms that collect HIPAA information will be required to reside in Qualtrics. If you are collecting other types of PII you will need to encrypt your form submissions. Let us know if you need any help with that after your account is created:
